Another way to collect participant information is by having the participant complete the Revolution Database Survey. If participants do not have time to provide their information over the phone, participants can be sent our survey to fill out at their convenience.

Study screeners should customize their surveys as similar to the Revolution Database Survey to simplify their study participant imports into Revolution



Information input into the survey will be saved in Qualtrics. To download the data, please follow the instruction below.

  1. Log in to Qualtrics. (Account information can be found in S drive)

  2. In the sidebar on the left, select "General DBM"


  3. On the right, you will see a survey named "Revolution Database Survey", click that survey.

  4. On the top, select a tab named "Data & Analysis".


  5. Use the checkbox on the left to select responses that you wish to export.

  6. Once the responses are selected, select "Export Data..." under a dropdown of "Export & Import".


  7. A popup will appear to have you choose what format you want to be downloaded, click Download


  8. Search if participant already exists in the database
  9. If not, manually create a new family and enter their information
