GATHERING INFORMATION
- Start filling out the General Information in the caller's name
- If you list them as, "Self," this indicates they are the "Head of Household"
- Researchers need to know basic participant information to greet them, identify who they are, confirm they are an adult, and determine it's appropriate to contact them again. Thus, First and Last Name are required to greet them, Date of Birth confirms the person is an adult, Sex helps us better identify the person, and the participant permission is needed to continue to contact them in the future.
- Required fields for creating the record are indicated in the list above with italics and asterisks, and within the form itself with asterisks
- Collect as much information as you can in order to determine whether families are eligible for various studies within the lab
- Should this not happen at the first point of contact with the family, there are two alternate ways to gather this information:
- (1) Calling the family and filling out the form with them
- Using the phone script (Interested Recruits Phone Script), you can call the family and then go through the form gathering their information
- (2) Emailing the family a link to the survey, such that they can fill out the form themselves
- Using the email script (Interested Recruits Email Template), you can email the family a survey link that will populate their information into Revolution
- (1) Calling the family and filling out the form with them
- Should this not happen at the first point of contact with the family, there are two alternate ways to gather this information:

- After you hit the "Create Record" button, Revolution will search through its database comparing this new record to previously entered records based on the participant's:
- First Name
- Last Name
- Date of Birth
- Sex
- If there is another record sharing the exact same information in all four of these criteria, you will be re-directed to the duplicate record

KR - I would put an example of what an address looks like, since I think many people may have never addressed a letter
