Participants can enter their basic contact information on buffalobehavioralmedicine.org or fill out the Qualtrics comprehensive Revolution Database Survey via 'Instant Sign Up' on their own.
Important Note
To officially add anyone to our database, the following information is required:
- First Name
- Last Name
- Date of Birth
- Sex
Future Contact
Remember, participation in research is voluntary. Participants must provide their consent to be input into our database for future contact.

Contacting Interested Recruits
Participants who only provide basic information will populate in the Interested Recruits table on the Revolution home page.
BEFORE contacting these people, check if they already existing in Revolution and/or Qualtrics survey. If they already exist in either, check the submitted date, delete their submission from the list.
To obtain more information, interested recruits can be contacted one of two ways:
- Phone - call the person with their provided phone number and use the Interested Recruits Phone Script to collect their information
- Email - email the person with their provided email address using the Interested Recruits Email Template to share our Revolution Database Survey
Important - check to see if the person already exists in the database before contacting them!
To prevent over contacting participants, users can delete an interested recruit submission under these conditions:
- After speaking to them
- After leaving them voicemail
- After sending them an email
- When they are already in the database
